Some Medical Practices are using this software as their administration software. Well this is fine since this is what it was designed for!. However practices who wish to use use this software for storage of DICOM images will not be satisfied with its performance!.

It sounds like the image attach feature was mainly intended for storing mug shots of the clients rather than their DICOM images. As they note, they really wouldn't want such images to be stored locally anyway (that's what a PACS database is for). They recommend to convert to JPG and store.
Image attachment would require a considerable amount of custom programming: traverse master client list creating the necessary directory structure, correlate the client list with their existing DICOM images, downsize those images, batch transfer to the created directory structure, etc. etc. It's quite a lot of work and the only sensible way to do it would be to automate the process as you wouldn't want to do that manually.
The better solution is the one they didn't recommend:
"Acquire a DICOM image viewer that is compatible with Microsoft
Internet Explorer. This will allow Capable 21C to display the DICOM images in the
Patient/Client Detailed Listings window."
Apart from the cost, it seems strange that they would rather recommend going the JPG path with unnecessary image duplication rather than use a DICOM viewer. Makes you wonder if the software's embedded browser control isn't very capable (probably size/scrolling restrictions).
